Defect

The mounting nuts used to attach the intermediate guide anchor brackets for the driver and front passenger seat belts were manufactured with a thread pitch that does not match the mounting bolts. As a result, the joint between bolt and nut does not engage correctly. If an extreme load is applied to the belt during a collision, the bolt may pull free from the vehicle body, causing the seat belt to fail to restrain the occupant.

Corrective action

Inspect the mounting nuts on all vehicles. If a nut with an incorrect thread pitch is found, replace it with the correct nut and install a new bolt.
