Filing in context
The Headlamp is the most frequently cited component category in this archive, appearing in 49 of its 4,890 filings. This filing is one of 39 Toyota notifications from 2021 in this archive. For scale, 3,640 of the 4,890 filings in this archive report a larger number of affected vehicles; the vehicles involved here were manufactured over a span of about 2 years and 8 months.
Defect
When used in taxi applications or other high-usage environments, the headlamp may remain illuminated for extended periods beyond the design assumption. Heat and ultraviolet radiation from the bulb may cause the aluminum vapor deposition on the reflector to peel. Continued use in this condition causes insufficient light concentration and a gradual reduction in luminous intensity. In the worst case, the headlamp may fail to meet the requirements of Safety Standards Article 32 (headlamp standards).
Corrective action
On all vehicles, replace the headlamp reflector with a corrected part and replace the lens with a new part.
