Filing in context
This filing is one of 19 Mazda notifications from 2019 in this archive. For scale, 4,693 of the 4,890 filings in this archive report a larger number of affected vehicles; the vehicles involved here were manufactured over a span of about 10 months.
Defect
Final inspection pass/fail determinations were made by unqualified personnel (inspection assistants) rather than certified vehicle inspectors, leaving the pass/fail process undefined. As a result, required Safety Standards inspections may not have been properly conducted.
Corrective action
All vehicles are to be inspected at a designated service facility and reviewed by a certified vehicle inspector. Any nonconformity with the Safety Standards is to be corrected. Inspection performed at an authorized repair facility, with correction of any identified nonconformity, constitutes a provisional measure.
