Filing in context
The Power transmission is the most frequently cited component category in this archive, appearing in 7 of its 4,890 filings. This filing is one of 24 Isuzu notifications from 2016 in this archive. For scale, 3,024 of the 4,890 filings in this archive report a larger number of affected vehicles; the vehicles involved here were manufactured over a span of about 5 years and 6 months.
Defect
On transit buses equipped with an automatic transmission, the torque converter backing plate mounting bolts are improperly torqued and the threaded portion of the torque converter cover is improperly machined, resulting in insufficient clamping force on the bolts. Continued operation causes the bolts to fracture. The fractured bolts become lodged between the torque converter cover and the turbine and, in the worst case, lock up the torque converter, stalling the engine and rendering the vehicle unable to be driven.
Corrective action
On all vehicles, replace the backing plate mounting bolts with new bolts and retorque them to the specified value.
