Defect

On aerial work platform vehicles, improper installation of the exhaust pipe creates a gap in the clamp tabs. Continued use causes the clamp bolt to fracture, allowing the exhaust pipe to shift and direct hot exhaust gas onto a nearby wooden jack base stored in close proximity. The jack base may char and, in the worst case, a vehicle fire may result.

Corrective action

On all vehicles, inspect the exhaust pipe clamp tightening condition. Where a gap is present at the clamp tabs, replace the clamp with a new part. Additionally, install a heat protector on the jack base storage compartment.
