Defect

The dimensional relationship between the handlebar holder and the handlebar post on the vehicle is inappropriate, leaving a gap at the handlebar holder seating surface. Vibration during vehicle operation causes one of the handlebar holder bolts to fracture. Continued use allows the remaining bolt to fracture as well, and in the worst case, the handlebar detaches from the vehicle.

Corrective action

On all vehicles, replace the handlebar holder with a new part, add a washer at the holder mounting point, and secure with a new lock nut.
