Filing in context
The Other Equipment is the most frequently cited component category in this archive, appearing in 32 of its 4,890 filings. This filing is one of 30 Toyota notifications from 2018 in this archive. For scale, 4,592 of the 4,890 filings in this archive report a larger number of affected vehicles; the vehicles involved here were manufactured over a span of about 2 years and 3 months.
Issue
On engine-powered forklifts, the bracket mounting the tilt cylinder — which tilts the outer mast forward and rearward — has insufficient strength. Repeated heavy-load lifting operations place excessive stress on the bracket, causing cracks to develop. Continued operation in this condition may result in the bracket fracturing and a complete loss of lifting function.
Corrective action
On all vehicles, replace the outer mast with a corrected part.
