Defect
On large and medium heavy trucks and buses, the criteria for identifying severe operating conditions (severe condition) were not clearly defined, making it difficult for owners to determine whether more frequent engine oil maintenance was required. If engine oil maintenance is not performed at the appropriate interval, the oil mist separator may become clogged with sludge — the accumulation of combustion soot and oil degradation byproducts. Clogging causes crankcase internal pressure to rise, forcing engine oil into the intake system. In the worst case, engine speed may increase unintentionally and engine damage may result.
Corrective action
On all vehicles, measure crankcase internal pressure. If the oil mist separator is determined to be clogged, replace it with a new unit and replace the engine oil and oil filter. On vehicles determined to have been operated under severe conditions, replace the engine oil and oil filter regardless of separator condition. Additionally, update the engine control software with the corrected program, which improves low-oil detection and adds engine overspeed prevention. Apply a sticker to each vehicle to promote proper engine oil maintenance intervals, and distribute a leaflet specifying the criteria for severe operating conditions.

状況
大型トラック及びバスにおいて、シビアコンディションの条件が判断し難かったため、適切なエンジンオイルのメンテナンスが行われないと、オイルミストセパレータがスラッジ（燃焼で発生する煤やオイル中の劣化物）の堆積によって目詰まりを起こす場合がある。そのため、クランクケースの内圧が上昇、エンジンオイルが吸気系に流入して、最悪の場合、意図せずエンジン回転が上昇し、エンジンが破損するおそれがある。
改善措置
全車両、クランクケースの内圧を測定し、オイルミストセパレータに目詰まりが生じていると判断されるものは、新品のオイルミストセパレータに交換するとともに、エンジンオイルとオイルフィルターを交換する。また、使用条件が厳しい（シビアコンディション）と判断された場合についてもエンジンオイルとオイルフィルターを交換する。さらに、エンジン制御プログラムに対策プログラム（オイル量減少時の検知性向上とエンジン過回転防止）を追加する。 また、適切なエンジンオイルメンテナンスの励行を喚起するステッカーを貼付し、シビアコンディションの条件を明記したリーフレットを配布する。
