Defect

On the tailgate loading unit of refuse collection vehicles, improper assembly control of the hook and U-bolt, combined with an incorrect hook shape, allows excessive load to be applied to the U-bolt during loading operations. This causes cracks to develop in the U-bolt. In the worst case, the U-bolt may fracture, causing the tailgate to spring upward and preventing the vehicle from accepting refuse.

Corrective action

On all vehicles, inspect the hook and U-bolt; if no clearance exists, replace the U-bolt with a new part (and on some vehicles, replace the hook with a corrected part) and reassemble the hook and U-bolt correctly. Additionally, distribute a revised owner's manual supplement adding tailgate lock inspection items, and affix caution labels to both the left and right sides of the body.
