Defect
In the automatic transmissions of ambulance vehicles, improper inspection procedures performed at the time of vehicle shipment allowed the torque converter to overheat abnormally, partially melting the oil pump bushing. Continued operation causes the bushing damage to progress, degrading seal performance and resulting in an oil leak. In the worst case, the vehicle may become inoperable.
Corrective action
On all vehicles, replace the complete automatic transmission assembly with a new unit and replace the engine-side automatic transmission oil seal with a new part.

状況
救急車の自動変速機において、車両出荷時に実施した点検方法が不適切なため、トルクコンバータが異常発熱して、オイルポンプブッシュの一部が溶損したものがある。そのため、そのまま使用を続けると、オイルポンプブッシュの損傷が進行してシール性能が低下し、オイルが漏れて、最悪の場合、走行できなくなるおそれがある。
改善措置
全車両、自動変速機一式を新品と交換するとともに、エンジンの自動変速機側オイルシールを新品と交換する。
