Defect
In the engine, two separate conditions may cause abnormal combustion. First, an improperly calibrated engine control program can cause abnormal combustion when the speed limiter activates during sustained high-load driving. Second, insufficient oil drainage performance inside the head cover — which is responsible for returning oil to the oil pan — allows oil pooled in the head cover to enter the combustion chamber as a mist carried by blowby gas during high-load hill climbing, causing abnormal combustion. In either case, the connecting rod may suffer fatigue fracture during driving, causing the engine to stall. If the fracture causes oil to be ejected, surrounding components may catch fire and the vehicle may become inoperable.
Corrective action
1. On all vehicles, reprogram the engine control program to the corrected specification. In addition, review vehicle operating data; on vehicles with extended high-load operating hours, replace the connecting rod assembly with new parts. Due to parts availability, replacement will be performed in order of priority, beginning with vehicles with the longest high-load operating hours. 2. On a portion of vehicles, install a corrected part (ventilator) on the head cover.

状況
原動機において、 1.エンジン制御プログラムが不適切なため、高負荷連続走行中に速度抑制装置が作動するような条件下で異常燃焼が発生することがある。 2.ヘッドカバー内油切性能(ヘッドカバー内のオイルをオイルパンに戻す)が不十分なため、坂路を高負荷走行するとヘッドカバー内に滞留したオイルがブローバイガスとともにミスト状の形で燃焼室に廻り込んで異常燃焼が発生することがある。 そのため、走行中にコンロッドが疲労破損してエンジンが停止し、コンロッドの破損によりオイルが飛散した場合は周辺部品が焼損し、走行不能となるおそれがある。
改善措置
1.全車両、エンジン制御プログラムを対策仕様に修正する。また、車両稼働データを確認し、高負荷での稼働時間が長い車両は、コンロッド一式を新品に交換する。なお、部品の準備に時間を要するため、高負荷での稼働時間が長い車両から順次交換作業を行うこととする。 2.一部の車両は、ヘッドカバー部に対策品(ベンチレータ)を取り付ける。
