Defect

On engines used in medium-duty fire apparatus, improper cleaning of the internal gears during manufacturing allowed foreign material to enter the gear shaft bushings. When the fire-fighting equipment is engaged and the engine is operated under load, the affected bushings may seize. In the worst case, the engine may stall and be unable to restart.
