Defect
In the engine used in mid-size fire trucks, inadequate cleaning of internal gears during manufacturing allowed foreign material to enter the gear shaft bushings. When firefighting equipment is engaged, the bushing may seize. In the worst case, the engine stalls and cannot be restarted.
