Defect
In the engine, the check valve body of the piston cooling oil jet assembly has insufficient rigidity. During assembly, the body may deform, preventing oil from being injected and leaving the piston without cooling. Continued operation causes the piston temperature to rise, leading to cracks or seizure. In the worst case, this may result in an oil leak and vehicle fire, or the vehicle may become inoperable.
Corrective action
On all vehicles, inspect the check valve function on a per-cylinder basis. If a malfunction is found, replace the complete piston assembly (piston, cylinder liner, and related components) for the affected cylinder with new parts. Additionally, replace the check valve on all vehicles with the corrected part. Because full supply of the corrected parts requires additional time, an interim measure applies: vehicles whose oil jet is confirmed to be injecting oil correctly will have the check valve replaced with the corrected part at a later date.

状況
原動機において、ピストン冷却用オイル噴射装置のチェックバルブボディの剛性が不足しているため、当該バルブを組み付けた際にボディが変形し、オイルが噴射されず、ピストンが冷却されないものがある。そのため、そのまま使用を続けると、当該ピストンの温度が上昇して亀裂又は焼き付きが発生し、最悪の場合、オイルが漏れ、火災が発生する。又は、走行不能になるおそれがある。
改善措置
全車両、気筒毎にチェックバルブの機能を点検し、作動不良があった場合、当該気筒のピストン一式(ピストン、シリンダーライナー等)を新品に交換する。また、全車両、チェックバルブを対策品に交換する。 なお、対策品の全数供給までに時間を要するため、暫定措置として当該オイルジェットがオイル噴射する場合には、後日、チェックバルブを対策品と交換する。
