Defect
In the fuel injection system, the injector assembly process is improper, resulting in excessive clearance between the plunger and barrel on some units. This causes the O-ring to deform and sustain damage, compromising the seal and allowing diesel fuel to contaminate the engine oil. Continued operation increases the oil level in the oil pan; diesel-contaminated oil is then drawn through the blowby hose into the intake manifold, causing engine speed to rise uncontrollably.
Corrective action
On all vehicles, replace the injectors with non-defective units. As an interim measure while parts are being sourced, inspect all vehicles for diesel fuel contamination in the oil. Vehicles with confirmed leakage are to have the injector replaced immediately with a non-defective unit; vehicles without confirmed leakage are to receive instructions for periodic self-inspection, with replacement by a non-defective unit to follow at a later date.

状況
原動機の燃料噴射装置において、インジェクタの組立て工程が不適切なため、プランジャとバレルの隙間が大きいものがあり、オーリングが変形、損傷して機密性が損われ、燃料の軽油がエンジンオイルに混入するものがある。そのため、そのままの状態で使用を続けると、オイルパンの油量が増大し、ブローバイホースを経由して吸気管に軽油の混入したオイルが吸入され、原動機の回転が上昇するおそれがある。
改善措置
全車両、インジェクタを良品と交換する。 なお、部品の供給に時間を要することから、当面の暫定措置として、全車両、オイル中への軽油混入を点検し、漏れているものは良品と交換し、漏れていないものは、定期的な点検方法を説明し、後日良品と交換する。
