Defect
On supercharged vehicles, the resin spacer used to mount the fuel delivery pipe is made of an unsuitable material. Heat causes the spacer to deform, loosening the mounting bolt, which may then fracture due to engine vibration. This compromises the seal at the injector mounting point, allowing fuel to leak. In the worst case, a vehicle fire may occur.
Corrective action
On all vehicles, replace the spacer and mounting bolt with corrected parts, and replace the injector holder with a new unit.
