Defect
On engine-powered forklifts, the bushing in the radiator cooling fan assembly may have insufficient crimping due to improper equipment parameter settings applied during production lot setup. This causes abnormal noise when the fan slides. Continued operation allows the bushing to fracture, causing the fan to detach and the engine to overheat. In the worst case, the engine may be damaged.
Corrective action
On all affected vehicles, replace the engine cooling fan with a conforming replacement unit.
