Defect
When the injector interior is abnormally worn — due to the use of low-quality fuel or similar causes — fuel injection volume increases, generating cracks in the piston that eventually perforate it. Combustion gas escapes through these perforations into the crankcase, raising internal pressure. This forces engine oil out through the rocker cover and, in the worst case, may result in a vehicle fire.
Corrective action
On all vehicles, replace the rocker cover with a corrected part incorporating an additional breather, and replace the rocker cover gasket with the corrected part, to prevent progression to a vehicle fire.

状況
インジェクタの内部が粗悪燃料の使用等によって異常摩耗すると、燃料噴射量が増大し、ピストンに亀裂が発生して穴が開き、この穴開き部分から燃焼ガスがクランクケース内に漏れて内圧が上昇する。そのため、ロッカーカバーからエンジンオイルが噴出し、最悪の場合、車両火災に至るおそれがある。
改善措置
火災に至らない様、全車両、ブリーザーを増設した対策品のロッカーカバーに交換するとともに、ロッカーカバーガスケットを対策品に交換する。
