Defect
The headlamp assembly has an improper structure that restricts heat dissipation. During operation, heat from the bulb and solar radiation causes the reflector to deform, tilting the bulb and shifting the optical axis. In the worst case, the solder at the socket melts, the lead wire pulls free, and the headlamp goes dark.
Corrective action
On all vehicles, replace the headlamp and socket with corrected parts, and replace the bulb with a new unit.
