Defect

On medium-duty buses, the specified tightening torque for the injector mounting bolts in the fuel injection system is incorrect, leaving the injector gasket with insufficient sealing performance on some units. Combustion gas leaks past the gasket, causing the injector mounting bolts to loosen. Increased vibration then causes the bolts to fracture, which stalls the engine and renders the vehicle inoperable.

Corrective action

On all vehicles, replace the gasket with the corrected part and tighten to the specified torque. If any fuel injection system components are found to be damaged, replace those parts with new ones.
