Defect
On the bracket that mounts the brake pedal to the vehicle body, the pivot shaft of the brake pedal may not be properly staked. Continued use allows the pivot shaft to pull free from the bracket. In the worst case, the brake pedal may detach from the bracket, making it impossible to apply the brakes.
Corrective action
Inspect all vehicles. On any vehicle where the pivot shaft is not properly staked, replace the bracket with a conforming part.

制動装置のブレーキペダルを車体に取付けている金具(ブラケット)において、ブレーキ ペダルの支持軸がかしめられていないものがあるため、そのままの状態で使用を続けると 、当該支持軸がブラケットから抜け出し、最悪の場合、ブレーキペダルがブラケットから 外れ制動操作ができなくなるおそれがある。
改善措置
全車両点検し、当該支持軸のかしめがされていないものは、ブラケットを良品と交換する 。
