Issue

The mounting structure for the upper flip-up door at the rear of the vehicle is inadequate. The plastic hinges along the upper edge of the door degrade and fracture due to atmospheric ozone, ultraviolet radiation, and similar environmental factors. When a hinge fractures, the door may detach and fall while open, posing a risk of injury to workers.

Corrective action

On all vehicles, install safety straps on the left and right sides of the flip-up door. In addition, inspect the hinges and replace any that show visible cracks with new units.
